All full time qualified employees (and their dependents), become eligible for group insurance benefits after they work a full calendar month. Enrollment in any of the benefit plans is Voluntary. If a person chooses to enroll in any or all of the plans they must submit an application for each of the benefit plans within the time frame determined by EIS. If an employee chooses to not enroll in the plans, they will need to complete the Decline or Waiver section of the enrollment forms and return them to Human Resources.

Once a new hire receives their on-boarding package of materials from Human Resources, they will be able to view the benefit details, along with costs for each.
